Sagging Breast Assessment — What Is Your Level of Breast Firmness?

"Breast ptosis is generally classified into three grades."

Grade I
The nipple is positioned at a level similar to the inframammary fold.
Grade II
The nipple is positioned below the inframammary fold, but remains above most of the breast tissue.
Grade III
The nipple is positioned below the inframammary fold and points downward at the lowest part of the breast.

※ Typically, the nipple is positioned 4-5cm above the inframammary fold, but when breast ptosis occurs, it descends toward the fold, which may require aesthetic correction.

In cases of mild breast sagging, a lifting effect may be achieved through breast implant augmentation alone. Incision placement can be customized through the axilla, inframammary fold, or areola depending on the patient’s anatomy and surgical plan.

Through a periareolar incision technique, excess stretched skin is removed and the breast tissue is repositioned higher on the chest wall. Breast volume remains largely unchanged while achieving a naturally lifted breast contour.

Vertical or anchor-pattern incision techniques may be considered depending on the degree of sagging. Breast lift surgery can also be combined with breast reduction to remove excess sagging tissue or with breast augmentation to enhance breast volume and shape simultaneously.
